The Position:

We are looking for a full-timeRN Clinical Supervisorwith experience in an office-setting, health-care profession, or related field. Job duties include but are not limited to:

  • Oversight of clinical activities, including patient assessments, individualized care plan development, care coordination, on-site field visits
  • Promote quality and participate in care coordination to ensure proper communication between caregivers, patients, referral sources and payers.
  • Provide oversight of delegation of the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies.
  • Be responsible for continual survey readiness
  • Complete all other documents required to provide in-home hands on service to client
  • Orient in-home agency staff to POC as needed
  • Review client status and document any changes to physical assessment, environmental assessment of POC
  • Evaluate skill level of new hires for in-home aide and skilled nursing positions as needed.
  • Instruct all staff as needed to provide education for unfamiliar tasks i.e. Hoyer lift operation
  • Receive and act to resolve complaints/concerns/issues from clients/families.
  • Nurses must be able to work independently and will supervise CNAs and caregivers, and other skilled nursing staff as needed to ensure appropriate and timely records of services being given
  • Performs or assists in procedural care requiring complex nursing skills and clinical judgment
  • Maintains and ensures client confidentiality at all times
  • Performs miscellaneous job-related duties as assigned (i.e. quarterly evaluations and client record reviews, employee annual evaluations)
  • Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel
  • Promotes home health referrals in the health care community

Are You the Right Candidate?

Please apply if you have the following qualifications.

  • RN License
  • RN - Minimum of 1 year previous experience with home care or hospital setting
  • Knowledge of Federal and State Regulations
  • Knowledge of Medicaid and daily operations of home care agency
  • Excellent Computer Skills
  • Hard work ethic, entrepreneurial mentality
