Regions Hospital is looking for an enthusiastic, creative and compassionate Paramedic with excellent communication skills to join our Community Paramedicine (CP) program. Our CP team delivers important meaningful care in patient's homes in order to help avoid hospitalizations and to improve our patient's quality of life. Join us and bring your EMS career to the next level! The role of Regions Hospital's Community Paramedic is to facilitate both acute care and chronic disease management in a patient's home. Our CP program served 375 patients and completed over 1200 home visits in 2019. This important program saved 600 hospital bed days by preventing admissions or readmissions and 100 % of patients report they would recommend the program to others. You will partner with EMS and hospital medicine physicians to prevent hospitalizations. Our team also partners with the patient's primary and specialty care providers to ensure chronic diseases are well managed in the home. The role is a great opportunity to utilize and further develop your critical thinking skills while continuing to use your paramedic skills. You will work under the medical direction of the EMS physician and carry out care plans approved by the EMS physician in partnership with the patient's primary care physician. This position will require travel within the Twin Cities metro area. If you don't yet have your Minnesota Community Paramedic certification, we will partner with you toward completion of the requirements.
