PRN JOB SUMMARY This position is responsible for assessments, treatments and clinical recommendations of patients with functional impairment and activities of daily living (ADLs) deficits according to physician's orders, scope of practice and bylaws from the American Occupational Therapy Association (AOTA). PRIMARY RESPONSIBILITIES: * Provides and conducts evaluation, diagnosis, treatment, education and clinical recommendations according to the scope of practice as an Occupational Therapist from the American Occupational Therapy Association (AOTA) addressing disorders effecting function and activities of daily living (ADLs). * Abides bylaws provided by AOTA. * Provides services in Occupational Therapy in accordance with the physician's plan of care. * Assists the physician in assessing the patient's level of functioning as it related to Occupational Therapy. * Instructs and advices patient, family and caregivers on maximizing patient safety and independence in all activities of daily living, including self-care, home management and independent living skills. * Demonstrates discipline specific competency in the skills required by the care center/facility. * Supervises and teaches the Certified Nursing Assistant (CNA). Communicates plan of care changes to the aide and other appropriate staff as necessary. * Performs a complete assessment of the patient, including screening for disorders impacting a patient's function and self-care. Based on assessment findings, develops a plan of care including specific interventions. * Addresses functional impairment due to physical, cognitive, psychological, and emotional etiologies. * Provides clinical recommendations based on patient need. * Observes signs and symptoms and reports changes in the patient's condition to the physician and/or other appropriate staff as necessary. * Participates and provides discipline specific instruction to patients, families, caregivers and staff. * Provides clinical recommendations and referrals to other physicians, clinicians, healthcare professionals or support services based on the patient and caregiver needs * Evaluates, and re-assesses needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care. Assesses the patient's continual care needs. * Conducts discipline specific patient screenings to determine clinical needs of the patient, medical necessity and skilled services. * Meets or exceeds documentation requirements for evaluation, short-term/long-term goals, re-assessment, daily notes and discharges. * Teaches, supervises and counsels the patient, family and caregiver regarding procedures and other care needs as appropriate to the patient's condition. Utilizes educational material as appropriate. * Develops, prepares and maintains individualized documentation on every patient with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. * Participates in continued professional development and enhancement of clinical knowledge through continuing education and peer reviewed literature. * Cooperates and consults with appropriate staff to provide staff education when requested. * Participates in department, care center, and company meetings as needed or requested * Provides in-services to other staff and outside referral sources, associations and/or support groups as needed or requested. * Provides clinical support and supervision of students and new graduate clinicians as needed or requested. JOB REQUIREMENTS: * Maintains professional licensure/certification according to state and national governing bodies. * Participates in continuing education courses for clinical development * Maintains current personnel file information and provides information to care center in timely manner. * Continue professional development to enhance clinical practice. Setting own development challenges and volunteers to learn. * Assists with orientation of new personnel. * Successful completion of an Occupational Therapy program at a school of Occupational Therapy approved by the American Occupational Therapy Association (AOTA) or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association. * Current and unencumbered license to practice Occupational Therapy specific to that state the employee is assigned to work by the Company. * One (1) year of Occupational Therapy experience preferred but not required. * Current CPR certification.
